Today’s life binder post will be on keeping your family’s medical records together and organized. I have 3 free printables that will help you do that.
The first is the Medical history printable. Print one copy for each family member (or create your own) & place it in your binder. This printable helps keep track of each family members medical conditions, medical history as well as name and contact information for doctors. My version is geared towards Canadians (since I am one) so there is a section for medicare number, if you do not have a medicare number where you live you may just ignore that section.
The second printable is the prescription information printable where you can keep track of your medications, the dosage, the number of refills you have remaining as well as any side effects.
The third and final printable in this set is the Doctor’s appointment tracker which allows you to keep track of why you go to the doctor, what the outcome is and what prescriptions/referrals/recommendations were made based on that visit. This is very useful in determining any sort of health patterns.
What other medical information do you keep or do you keep any at all?
That is actually a really smart idea. My kids pediatrician is online, so I can get their info anytime, but I don’t have access to mine.

I love that you provided the printables! This is a really good idea! My mom would hold all of our things together in a binder and I need to keep all my information together better because right now it’s in a drawer.

I am still within my ten year frame for breast cancer, so I keep a ton of data and information. I like the organizational tools you are suggesting, though. I am all about a good printable.

Being organized like this is definitely a sure way to keep on track with everything! Especially as we get older and need a little more medication/visits to the doctor.

This is one part of our binder that doesn’t exist! We have an interconnect doctor’s network that is pretty snazzy, but it would do to have records of our own!

I kept a medical binder many years ago, when I was in high school. Going back to this is a great idea. So many records could be kept there and I’d have them for years instead of months before they got lost!

This is a great idea! My aunt who was a midwife before she retired, and uncle who is a retired RN did this for my cousin. She has this medical binder full of everything that she went through as a child…it’s just a great idea!

As an ER nurse, I truly appreciate a complete written history and list of current -current being the key- list of medications. Everyone should have this section in their home binder!

This is a great idea – whenever I have to see a new doctor – I always have to write down all my medications and try to keep track of it all. Like what has worked in the past and what works now

I need that Doctor’s Appointment printable a few months ago when I was pregnant! Between my appointments and my kid’s, it was so hard to keep track!

I keep all of our medical stuff in a folder in my office filling cabinet! I have no clue what’s in there but if something ever happened the proof is there lol

I have never really thought of printing out our medical information, but definitely a brilliant idea. Things can happen and whatever it may be it is good to be prepared ahead of time. Thank you for sharing this to us. xx
